The Therapy Factory, LLC, is an out of network practice. The client is responsible for the fee on the day of your session. We provide you with a superbill that you can submit to your insurance company for a reimbursement. If you want to estimate what your insurance company will cover, you may want to call and ask:

• Do I have an out of network benefit if I see an out of network provider for therapy?

• Do I have a deductible for out-of-network services?

• Do I need a pre-authorization for out-of-network services?

• How many sessions will the insurance company authorize before a treatment plan is required?
